TAX PREPRATION CHECKLIST

The information that is needed to prepare your taxes is dependent on each individuals personal situation as well any changes that may have occurred during the year. 

The checklist below list common forms and documents that we will need to get you started.

PERSONAL INFORMATION

  • Photo Identification (Driver’s license, State I.D., Passport etc.)
  • Social security card or Tax ID
  • Current Address (If different from photo ID)

DEPENDENT INFORMATION
(If applicable)

  • Social Security Card or Tax ID
  • Birth Certificate
  • Childcare records (if applicable)

SOURCES OF ALL INCOME RECEIVED

Common sources include:

  • W-2
  • 1099 (Independent contractors)
  • Unemployment (1099-G)

Self Employed

  • Records of all Income and Expenses
  • Records of estimated tax payments made (if applicable)
  • Assets used for the business
  • Business use of the home information (home based businesses)

Retirement Income

  • Pension/IRA/Annuity income (1099-R)
  • Social Security/RRB income

Rental Income

  • All records of income and expenses.
  • Rental Asset information
  • Record of estimated tax payments

Investments/ Savings

  • Interest, dividend income (1099-DIV, 1099-INT)
  • Income from sale of stock or other property (1099-B, 1099-S)
  • Investment Expenses
  • Record of estimated tax payments made (Form 1040-ES)
  • Health Savings Account reimbursements (1099-SA)

Deductions

Deductions are expenses that could potentially lower your taxable income. These deductions are based on your individual life situation.

Charitable Donations

  • Amounts donated to any charitable organization (Schools, churches, etc.)
  • Records of non-cash charitable donations

Home Ownership

  • Mortgage Interest Statements (Forms 1098)
  • Personal property tax records and real estate records
  • Any other 1098 forms

State & Local Taxes

  • Records of any state/local income tax paid/state and local sales taxes paid
  • Invoice for vehicle sales tax

Health Insurance

  • Form 1095A if enrolled in an insurance plan through the marketplace (exchange)

K-12 Educator Expenses

  • Expenses relating to classroom expenses incurred by educators for grades K-12

Educational Expenses

  • Form 1098-T from College/University
  • Records and receipts for qualified additional expenses
  • Form 1098-E if you paid student loan interest

Childcare Expenses

    • Receipts for fees paid to a licensed child daycare center or aftercare
    • Wages paid to a baby-sitter
